Post Hospital Admission Rehabilitation


Private One to One Occupational Therapy service for individuals recently discharged from hospital, or who have experienced a decline in mobility or function. this service aims to restore confidence, mobility and independence.

Support includes:

• Rebuilding strength after deconditioning
• Safe progression of walking, transfers, and daily activities
• Cognitive assessments and strategies to support a decline in memory
• Falls risk screening and personalised recommendations

Strength & Balance Training Sessions


Targeted programmes to build resilience, reduce falls, and maintain independence.

• Progressive strength training (seated or standing)

• Education on how strength and balance protect long‑term independence

• Balance and strength training

• Education on footwear, hydration, and medication awareness
• Carer training on safe mobility assistance
• Early warning signs of decline and what to do

Community Outcome‑Focused Sessions


Evidence‑based group sessions designed to improve measurable outcomes and reduce avoidable hospital admissions.

• Functional outcome measures (e.g., TUG, 30‑second sit‑to‑stand, gait speed)
• Regular progress tracking to demonstrate improvement
• Strength, balance, and mobility training tailored to older adults
• Education on hydration, activity levels, and daily movement habits
